White letter size paper order form used in the late 1960s by Oscar C. Howard's Meals on Wheels program in Minneapolis, Minnesota. "HOWARD'S CATERING-CAFETERIA/ 'Meals on Wheels'--Deluxe Service of Fine Foods/ 3300-3302 Fourth Avenue South, Minneapolis, Minnesota 55408/ Phone: Taylor 4-6994" printed at top. "Meals on Wheels" is stamped in red ink in top corners; below are order forms for entrees, etc., for Monday-Saturday.
